Then the doctors meet Shannon, a woman whose bad nose job left her with a witchy nose. After returning to her former surgeon for help, she still wasn't happy with the final result. But after meeting with Paul, she is given hope that she will no longer have to look like the wicked witch of the South.

Over the years, her nose has became less symmetrical, so to improve her looks and feel deserving of her husband, she turns to the famed plastic surgeons for help.

They meet Luis, a man who wants to fulfil his dream of becoming a space elf. However, the doctors warn that he could lose his vision in the process so he has a difficult choice to make when considering his future. The doctors then meet Stacie, an accident-prone woman, who is hoping to receive her 15th nose job. However, as she had so many operations before, there is the chance that the skin of her nose could turn black and die. Despite this risk, Stacie decides she wants to have the procedure. Terry and Paul also get to know Leslie, a former bodybuilder who needs the doctors to fix her painful, displaced boobs. Her problems were caused by some seriously shifty implants but with the help of the show's doctors, she wants to remedy the situation and enjoy life with larger breasts that stay in place.
A childhood injury left Nasreen with a crooked nose. As an adult, she visited a doctor who promised to help her. However, he botched the operation and Nasreen now needs major reconstruction. She turns to Terry and Paul for help, not only to fix her nose but to cure her crippling panic attacks too. The doctors then meet Luke, a transgender man who hopes Terry will be able to fix his dented chest. His problems arose after having surgery to make himself look more masculine, but the operation was botched and his chest now looks worse than it did originally. Terry promises to come up with a solution to a seemingly unfixable problem, as he tries to help Luke become a man who looks the same on the outside as he feels on the inside. The doctors also reunite with Martina Big, a previous patient on the show, whose extreme summer tan injections have led her back to the plastic surgeons. Season finale.
In the season premiere, doctors Paul Nassif and Terry Dubrow are reunited with a familiar face. Rajee, who sought the help of the famed plastic surgeons last season, is back. It had been decided that an operation to repair her cement-filled face was too dangerous, but after considering her case Paul and Terry come up with a solution. Rajee isn't the only person on their case list. They also meet Melissa, a woman whose nipples are too high. Feeling self-conscious because of the way her breasts look, she agrees to a boob job to remedy matters. Then they have a close encounter with Vinny, an 'out of this world' patient who wants to look like an alien. For this to happen, he wants his nose straightened and the bridge of his nose widened. He also wants smaller lips. The doctors come to his aid, although they are more concerned with his ability to breathe than his dreams of looking like an extra-terrestrial being.

She must have predicted what their response would be because it's good news that she receives. Also on the show is Ann-Marie, a cancer survivor, who hopes Terry will be able to reconstruct her breasts and abdomen. While she has accepted her scarring, she also wants to look in the mirror again and not cringe. Thankfully, she is able to move on with her life thanks to Terry's intervention.

While she should have been asleep during the procedure, she was actually awake and she heard and felt everything during the surgery on her nose. As the operation wasn't successful anyway, she has to brave more surgery, but this time it's under the safe hands of Terry Dubrow and Paul Nassif. The famed plastic surgeons also meet the late actress and music video vixen Tawny Kitaen ('Bachelor Party;' 'Hercules: the Legendary Journeys') who wants to get rid of her breast implants after a botched operation. However, they need to get the blood flowing to Tawny's nipples after her surgery and their method for doing so is more than a little surprising for the former sex symbol. Viewers with a fear of leeches might want to avert their eyes. Also this episode, the doctors meet a Brazilian 'doll' who wants a bigger butt.
