The doctors are out of the office and on the road to Montana to make a house call on a patient who, as a teenager, went head first into a chain link fence in a horrific motorcycle accident. Heather suffered multiple facial injuries as a result of her collision and after having the stitches removed following her operation, she believed she looked like a monster. Several reconstructive surgeries later, she still has problems with her appearance, as well as her breathing, and so she turns to Terry and Paul for help. By improving her appearance, she will finally have the confidence to go outside without the fear of public embarrassment. Also on the show is Sonya, who needs a plastic surgery makeover after experiencing problems with her stomach and breasts. She hopes to return to her sassy outgoing self after the op, so the doctors do what they can to help her. Outside of work, the expert surgeons decide to do a spot of fly fishing in the great outdoors.

Terry and Paul have encountered many strange cases during their time in the plastic surgery profession, but this episode features a case that is stranger than most. They meet Luis, a man who wants to fulfil his dream of becoming a space elf. However, the doctors warn that he could lose his vision in the process so he has a difficult choice to make when considering his future. The doctors then meet Stacie, an accident-prone woman, who is hoping to receive her 15th nose job. However, as she had so many operations before, there is the chance that the skin of her nose could turn black and die. Despite this risk, Stacie decides she wants to have the procedure. Terry and Paul also get to know Leslie, a former bodybuilder who needs the doctors to fix her painful, displaced boobs. Her problems were caused by some seriously shifty implants but with the help of the show's doctors, she wants to remedy the situation and enjoy life with larger breasts that stay in place.

A British bum ambassador is packing on the pounds to get a Kim Kardashian-inspired butt. She believes she could be the United Kingdom's version of the reality star, and hopes the doctors will agree to another butt-lift. But their opinion is that it would put her health at risk. Meanwhile, Dr Dubrow helps a mother of nine battle one of the worst cases of capsular contracture he's ever seen. Jessie had enlarged her breasts when younger, but got persuaded into getting much larger ones than she had initially wanted. Now, her breasts are asymmetrical, and she needs the doctors to fix that. Dr Nassif is bribed with baklava to help a Lebanese pop star fix his nose. After breaking his nose in an accident, Sammy had work done to fix it, four different times, but he still has problems with it, including having trouble breathing. All three have sought the doctors to fix their botched plastic surgeries.
