The plastic surgery experts meet Krystina, a British woman with heart-shaped areolas who wants to look like a caricature of herself. This is inspired by a drawing she has seen and she surprises Terry and Paul with her unusual request. Caricatures should be done on drawing boards and not on operating tables, Terry says, as she considers the implications of a procedure. Danielle is also on the show, the sister of singer Christina Milian, and she wants the doctors to fix her scarred stomach and popped implant. She has a complicated medical history, as well as a tragic one, but Paul is able to help her emotionally by healing her physically. The doctors then meet Cindy, a police officer who wants the doctors to fix her pug nose. By doing so, not only will they improve her appearance but they will make her look more like her twin again too. A previous operation went wrong, but Paul is able to give the woman the nose that she has been hoping for.

The doctors are out of the office and on the road to Montana to make a house call on a patient who, as a teenager, went head first into a chain link fence in a horrific motorcycle accident. Heather suffered multiple facial injuries as a result of her collision and after having the stitches removed following her operation, she believed she looked like a monster. Several reconstructive surgeries later, she still has problems with her appearance, as well as her breathing, and so she turns to Terry and Paul for help. By improving her appearance, she will finally have the confidence to go outside without the fear of public embarrassment. Also on the show is Sonya, who needs a plastic surgery makeover after experiencing problems with her stomach and breasts. She hopes to return to her sassy outgoing self after the op, so the doctors do what they can to help her. Outside of work, the expert surgeons decide to do a spot of fly fishing in the great outdoors.

They then meet 'cougar' Lisa who has a 'cobra neck deformity' following a face lift. After surgery, there is the possibility that her appearance will return to how it was pre-op, but she is willing to take the risk if it will help her to look younger. Also on the show is Whitney, a desperate patient who wants the plastic surgery experts to downsize her triple D breasts. She previously saw a surgeon for a reduction and a lift but during her procedure, he also gave her implants. This was against her wishes and she now wants Terry and Paul to put right what the previous surgeon did to her. According to Terry, Whitney's problem is deceptively difficult as it is more complicated than most. However, no job is too big or too small for the celebrated doctor and he does what he can to improve the appearance of her breasts.

This season premiere introduces Californian doctors Terry Dubrow and Paul Nassif, who specialise in undoing the damage caused when plastic surgery goes wrong. By their reckoning, at least half of cosmetic procedures are performed by the ill-equipped. They make a house call for Justin Jedlica, a man who is, through multiple surgeries, attempting to turn himself into the human equivalent of a Ken doll. He tells them about his ongoing plans for more and more operations and seeks their opinion. But whether Justin heeds their words as he pursues the perfect face and body remains to be seen. The doctors also meet a woman who's been left with a uni-boob after badly bungled breast surgery. Then, in a reminder that not everybody seeking cosmetic enhancements is doing so out of a sense of vanity, they take on the case of an actress who's undergone six nose jobs. But in her case, it is the agonising ongoing result of having been in a car accident years earlier.

A wealthy UK patient with a duck butt and bulging belly forces Dr Terry Dubrow to perform a two-stage revision to restore her 24-year-old body. And later, a mother of three prays her nasal problems are not life threatening. After getting a nose job as a present from her parents for her graduation, she has since faced significant difficulties as the operation did not heal properly. She goes to see the doctors, who are worried that the problems with her nose might be something more serious. Three plastic surgery operations that went wrong, and which the doctors must now fix.

A British bum ambassador is packing on the pounds to get a Kim Kardashian-inspired butt. She believes she could be the United Kingdom's version of the reality star, and hopes the doctors will agree to another butt-lift. But their opinion is that it would put her health at risk. Meanwhile, Dr Dubrow helps a mother of nine battle one of the worst cases of capsular contracture he's ever seen. Jessie had enlarged her breasts when younger, but got persuaded into getting much larger ones than she had initially wanted. Now, her breasts are asymmetrical, and she needs the doctors to fix that. Dr Nassif is bribed with baklava to help a Lebanese pop star fix his nose. After breaking his nose in an accident, Sammy had work done to fix it, four different times, but he still has problems with it, including having trouble breathing. All three have sought the doctors to fix their botched plastic surgeries.
